Help is on the way
For over 47 years, James and Sandy have been inseparable. Their deep love and devotion have enabled them to overcome many hardships. Since their grown children moved away, they only have each other for support.
James’ health has rapidly declined since being diagnosed with dementia. He now needs help with everyday activities, including moving around their home. Sandy, who also has health issues, is finding it more difficult to manage James’ medications and appointments. The doctors recommend moving James to a nursing home, but neither can stand the thought of living apart. Sandy is desperate to find an affordable solution so they can remain together in their home.
You or someone you know may be facing similar challenges. Nursing homes play a crucial role in our community by providing care for individuals with complex conditions who can no longer safely live at home. But what about those who just need extra help so they can still live at home?
There is a new level of care coming to Brevard
St. Francis Hospice Lifestage has been approved by the Centers for Medicare and Medicaid Services to open the first and only PACE Center in Brevard County. The Program of All-Inclusive Care for the Elderly (PACE) will play a vital role in supporting individuals who would otherwise rely on costly nursing home care funded by Medicaid.
PACE will provide all of a patient’s medical care by a collaborative team of medical experts. Together, the team will implement a personal care plan that will be regularly updated to address the patient’s changing medical and social needs. PACE will even include services like transportation, social activities, caregiver support, and respite care.
To qualify:
- Age 55 or older
- Certified by the state of Florida to need nursing home level of care
- A resident of Brevard County
- Able to live safely in the community
- Be able to continue to live at home without jeopardizing health or safety
- Be dually eligible for Medicaid and Medicare, or Medicaid-only
The PACE Center will include:
- Medical Services: Primary and specialty care, physical therapy, occupational therapy, and nursing services tailored to individual health needs.
- Social Services: Counseling, social work support, and assistance with accessing community resources to address emotional and practical needs.
- Nutrition and Wellness Programs: Nutritious meals, dietary counseling, and fitness programs to promote physical health and well-being.
- Day Programs and Activities: Engaging social activities to reduce isolation and increase quality of life.
- Transportation Services: Reliable, accessible transportation to medical appointments and other necessary outings.
- Caregiver Support: Resources, respite care, and educational programs to support family caregivers in their crucial role.
